UMPIP Contribution Election Form

This form authorizes the plan sponsor/salary-paying unit to deduct a specified amount/percentage from the participant’s pay to be contributed to the United Methodist Personal Investment Plan (UMPIP) on a before-tax, Roth or after-tax basis (for contributions on or after January 1, 2021). Completed, signed form is returned to the plan sponsor/salary-paying unit.
